РЕФЕРАТ
Появление в организме пациентов с гематологическими заболеваниями агрессивных форм железа представляет потенциальную опасность для жизни. Перегрузка организма железом связана с регулярными переливаниями донорских эритроцитов и/или повышенным всасыванием железа в кишечнике, а также повреждающим действием цитостатических препаратов. Цель исследования — выявить больных, нуждающихся в хелаторной терапии. Проведен анализ результатов скринингового обследования 727 пациентов с различными гематологическими заболеваниями. Максимальная концентрация ферритина выявлена у больных с талассемией, первичным миелофиброзом (ПМФ) и миелодиспластическими синдромами (МДС), которым было проведено наибольшее количество трансфузий эритроцитной массы: 80, 37 и 35 доз соответственно. Наименьшая концентрация ферритина была у больных хроническим лимфолейкозом и гемолитической анемией, которым было перелито 18,5 и 16,5 доз эритроцитов соответственно. Выявлена корреляция между уровнем ферритина и общим числом перелитых доз донорских эритроцитов: r = 0,462; p = 0,000. В онкогематологии кандидатами на хелаторную терапию в первую очередь являются больные МДС и ПМФ, у которых высокий уровень ферритина отражает избыток посттрансфузионного железа.
