ABSTRACT

The article focuses on the technical aspects of splenectomy (SE) in 1628 patients using two main techniques: laparoscopy (885 SEs) and laparotomy (743 SEs), with a description of various factors that influence the invasiveness and the success of the surgery. The fact is that in addition to the shape and size of an enlarged spleen, the technical complexity of the SE, especially in laparoscopic access depends on the following factors: perisplenitis, close presentation and fusion of the tail of the pancreas to the spleen, tumor infiltration of splenic vascular pedicle, branchy type of its structure, visceral obesity, compression of the splenic pedicle with enlarged and united lymph nodes. In most cases (60 %), there is a combination of several risk factors, thus making the surgery even more difficult. The indications for splenectomy, as well as its influence on the course of the main hematological disease are not considered in this paper.

ABSTRACT

This report describes major events of the 70-year history of the Hematology Clinic of the Russian Scientific Research Institute of Hematology and Transfusiology founded in 1945. It covers activities related to studies of etiology, pathogenesis, diagnosis, and therapy of hematological disorders, as well as training of the personnel and development of methods for improvement of the system of specialized medical care for hematological patients in the Russian Federation. Much attention is paid to personnel’s faultless work. The article emphasizes that recent scientific discoveries are successfully implemented in the everyday clinical practice and are published in leading domestic and foreign periodicals. It makes a special emphasis on modern therapeutic and diagnostic equipment that allows to apply all modern methods of diagnosis and therapy of hematological disorders.

Diagnosis and treatment of mental disturbances in patients with hematological disorders

ABSTRACT

Mental disturbances emerged in patients with hematological disorders are discussed. Such conditions are categorized into somatogenic psychoses (n = 118) and psychogenic (nozogenic) reactions (= 192). A subgroup of somatogenic psychoses consisted of delirium (n = 73; 61.9 %) and endogenomorphic psychoses (n = 38; 32.2 %) which included hallucinatory-paranoid states (n = 19; 50 %), depressive-delusional states (n = 16; 42.1 %), oneiroids (n = 3; 7.9 %), and psychogenically-induced schizophrenia episodes (n = 7; 5.9 %). A subgroup of nozogenic reactions included neurotic states: anxiety-dissociative (n = 67; 34.9 %), anxiety-phobic (n = 42; 21.8 %), affective (depressive-anxiety, n = 51; 26.6 %, hypomanic, n = 3; 1.6 %), schizophrenic (dissociative, n = 19; 9.9 %, anxiety-coenestopatic n = 10; 5.2 %). Therapeutic techniques for relief of symptomatic psychoses and nozogenic reactions in the studied patients were developed. These techniques included administration of psychotropic agents prescribed depending on the clinical presentation of the disorder and possible side effects, as well as the psychotherapeutic methods aimed at improving patients’ adherence to treatment.
